Five Faces gives you a private reference-based place to recognise, reflect, track, prepare and see progress - then share only through confirmed service connections.

A GP practice, counsellor, therapist, social worker or other participating service becomes visible only when a connection to your reference has been confirmed. One reference may be connected to more than one service to support continuity.
Messages, activities, appointments and provider updates are labelled by the service that supplied them. Full provider-specific detail can be opened from the central journey view.
The record exists to make the next conversation more useful, not to replace it. Five Faces is not a diagnostic or treatment system.
